OpScan Form Request

FEMA's Independent Study Program (ISP) has implemented new OpScan answer sheets, Form 95-23. Prior editions of the OpScan forms are no longer accepted and any expired forms submitted to ISP will be returned to the sender. The 95-23 OpScan forms can be obtained by filling out the request form at the bottom of the page. OpScan form requests will be processed and mailed by USPS media mail. Please allow 4-8 weeks for delivery.

Instructors conducting classes and sending groups of OpScan forms must also submit a single copy of the test questions from the administered exam and the completed Independent Study Delivery Processing Form. Opscan forms submitted without both of these items will be returned to the sender.

The current version of classroom exam questions must be used by the instructor. Please see the group delivery page for each course for instructions on obtaining the most current version of the exam.
